Reablement Service Practitioner

As a Reablement Service Practitioner , you’ll be working across all disciplines within the Reablement service including occupational therapy and the community response team (CRT). You’ll be part of a progressive team, using an enabling, adaptive approach, supporting people to live in their own homes
